What is Integrated Care?

Integrated Care simply means that your physical health and emotional health are
part of “whole person care.” At Genesee Community Health Center (GCHC), we look at you and your needs, and work with you on a plan “For You.”

A Patient-Centered Medical Home is a place where medical and behavioral health clinicians work together with you to address your concerns. GCHC is a nationally recognized patient-centered medical home.
